A red face doesn’t make it Fire
Liver-Yang rising and Liver-Fire blazing share the headache, the temper and the red face. The exam puts them side by side and waits for you to decide on a sign they share. Here is how to decide on a sign that belongs to only one of them.

The trap
A case often opens like this:
A 55-year-old man has temporal headaches, a red face, red eyes and a short temper. Pulse: Wiry.
Many students stop here and choose Liver-Fire.
But every sign in that sentence can also belong to Liver-Yang rising. The stem has not told you the answer yet. It has only told you that the Liver is sending too much upward.
Why it catches people
We learn “red means Heat” early, and it is often true. But a red head is not proof of Fire.
In Liver-Fire blazing there is real Heat, and it shows all through the body. In Liver-Yang rising, Yang goes up because the Yin or Blood below is short. FCM calls it a Full/Empty pattern with no real Heat: the top looks busy, the bottom is empty. Chinese textbooks (DTCM, CAM) even give Liver-Yang rising a flushed face, red eyes and a red tongue, so the head alone cannot decide.
Sort the clues
Put every sign in the stem into one of three boxes. Only the outer two boxes can decide.
Empty below
Points to Liver-Yang rising
- Aching, weak lower back and knees
- Top-heavy: head heavy, legs light
- Thready pulse (Wiry-Thready)
- Pale tongue (Blood root) or peeled tongue (Yin root)
Shared
Don’t decide on these
- Headache at the temples
- Dizziness, tinnitus
- Irritability, outbursts of anger
- Red face, red eyes
- Wiry pulse
Heat all through
Points to Liver-Fire blazing
- Thirst
- Dry stools, dark-yellow urine
- Bleeding (nosebleed, blood in vomit or sputum)
- Red tongue, redder sides, dry yellow coating
- Excessive-Wiry-Rapid pulse
Pulse names follow the Candidate Handbook (Appendix G). FCM writes Fine for Thready and Full for Excessive.
Three checks before you answer
Cross out the shared signs
Headache, dizziness, temper, red face and eyes, Wiry pulse. They tell you “Liver, going up”. They don’t tell you which pattern.
Look down: is the root empty?
Weak back and knees, a top-heavy feeling, a Thready pulse, a Pale or peeled tongue. If you find them, think Liver-Yang rising.
Look through the body: is there Full-Heat?
Thirst, dry stools, dark urine, bleeding, a dry yellow coating, an Excessive pulse. If you find them, think Liver-Fire.
If the stem gives you only the tongue, use FCM’s shortcut: Red tongue + redder sides + dry yellow coating is enough for Liver-Fire.

Three questions with the same trap. Pick an answer to see the explanation.
Question 1 of 3 · Easy
A 62-year-old woman has had headaches at the temples and dizziness for a year, worse when she is upset. Her cheeks flush easily and she is irritable. Her lower back and knees ache and feel weak, and she says her head feels heavy and her legs light. She is not thirsty, and her stools and urine are normal. Tongue: red, dry. Pulse: Wiry-Thready. What is the most likely pattern?
How to reach it
Cross out what both patterns share: temporal headache, dizziness, irritability, flushed cheeks, a red tongue. Now look down. Her back and knees are weak, her head feels heavy over light legs, and the pulse is Thready. The root is empty. Then look through the body: no thirst, normal stools and urine, no yellow coating, so there is no Full-Heat. A busy top over an empty bottom is Liver-Yang rising, here on a Yin-deficient root.
Other options
- A) Fire would add thirst, dry stools, dark urine, a dry yellow coating and an Excessive pulse, with no weak back or knees.
- C) Would lead with distension and mood. No empty root, and the tongue is not red.
- D) Would give blurred vision, a Pale tongue and a Choppy pulse, not a red face over weak knees.
Remember
Red top + empty bottom + Thready pulse = Liver-Yang rising.
“aching pain and weakness of loins and knees, top-heaviness” (DTCM 2.3.4.5)
Question 2 of 3 · Easy
A 40-year-old man has a splitting temporal headache, red eyes, a bitter taste and great thirst. He has not opened his bowels for three days, his urine is dark yellow, and he had a nosebleed this morning. Tongue: Red, redder on the sides, dry yellow coating. Pulse: Excessive-Wiry-Rapid. Which treatment principle fits?
How to reach it
The headache and red eyes are shared, so set them aside. Look through the body: thirst, three days without a bowel movement, dark urine, a nosebleed. That is Full-Heat everywhere. The tongue alone would be enough: Red, redder sides, dry yellow coating. Nothing is empty below. This is Liver-Fire blazing, so the principle is to clear the Liver and drain Fire.
Other options
- A) For Liver-Yang rising, which needs an empty root and has no Full-Heat through the body.
- C) For Liver-Qi stagnation: distension and mood, no Heat signs.
- D) For Liver-Blood deficiency: Pale tongue, Choppy pulse.
Remember
Heat all through + dry yellow coating = Liver-Fire. Main point LR 2, reducing, no moxa.
“A Red tongue with redder sides and a dry yellow coating would be enough to diagnose Liver-Fire” (FCM Ch.34)
Question 3 of 3 · Easy
Which finding best tells Liver-Yang rising apart from Liver-Fire blazing?
How to reach it
Ask which finding belongs to only one pattern. Weak back and knees with a Thready pulse show an empty root below, and only Liver-Yang rising has one. Liver-Fire is Full, so its pulse is Excessive, not Thready.
Other options
- A) Both patterns give temporal headaches.
- B) The trap itself. Liver-Fire has them, and DTCM and CAM give them to Liver-Yang rising too.
- D) Both pulses are Wiry. What is added to Wiry decides: Thready or Excessive-Rapid.
Remember
Decide on what only one pattern has. Shared signs never decide.
Compare FCM Table 34.2 with DTCM 2.3.4.5.
